For Athletes

Athlete Medical Release Form & Parent Authorization (200)

All athletes are required to have a completed medical authorization form and parent release form (where applicable) on file before participating in ANY practice or event.

Athlete Code of Conduct (130)

All athletes are expected to abide by this Athlete Code of Conduct.  Please print it out and familiarize yourself with the document.

Sport-Specific Forms

For forms, rules, and regulations specific to a sport that an athlete is interested in, please visit the Sports Offered page on the SONC website and click on the sport you are interested in.  The available forms and downloads will be listed within the post for that sport and on the right hand side of the page.  These forms are updated periodically, and visiting the main North Carolina Special Olympics website to receive them will ensure you have the most current versions.

For Coaches

Coaches Code of Conduct (105)

The Coaches Code of Conduct is an important document to remind past, present, and future coaches about the role of the coach in the athletes experience with Special Olympics.  Also, bear in mind the Coach’s Oath, recited by an SONC coach during  Opening Ceremonies at all state-level events.

“In the name of all coaches, we shall follow written and verbal instructions of Special Olympics officials at all times, have our athletes at the appropriate events and activities at the proper time and abide by the rules and policies, in the spirit of sportsmanship.”

For Volunteers

Volunteer Code of Conduct (136)

We ask all volunteers to read and sign this document, to ensure the role and expectations of a volunteer are understood.

Volunteer Bill of Rights (151)

Each volunteer is a valuable member of our program, without whom we would not be able to support our athletes.  We believe every volunteer is entitled to respect and decency, and we offer a Volunteer Bill of Rights to help support this view.

Unified Sports

Unified Partner Form (92)

This form is for all volunteer athletes wishing to participate alongside our Special Olympians in Unified Sports.  It must be completed prior to any participation.
